Offering more than competitors but coming it at $400, Razer’s latest device looks set to push cloud gaming to the edge. The new Android-powered console supports all the major game launchers you could think of. Epic Games, Xbox Cloud Gaming, Nvidia GeForce Now, Steam Link, Moonlight, and Parsec can all come with you on the go. From day one, the Razer Edge will boast a sizeable catalogue of games, including your mainstream favourites. It houses a monster 6.8-inch AMOLED display, with a 144Hz refresh rate and a 2400×1080 Full HD+ resolution. With a Gorilla Glass coating, it’s drop and scratch resistant as well. This screen impresses more than competitors such as the Steam Deck and Logitech G Cloud, both of which have a much lower refresh rate. Plus, it’s lighter than the Steam Deck and Logitech G Cloud at just over 400g. Surrounding the display is Razer’s Kishi V2 Pro controller with analogue triggers, microswitches, programmable buttons, and an “ultra-precise” D-pad. It also packs in haptic feedback and a 3.5mm headphone jack. You can purchase the controller separately for Android devices, too. Inside the handheld console, you’ll find the Snapdragon G3x Gen 1 gaming processor, designed especially for the Razer Edge. It’s a beast of a chipset with a 3Ghz octa-core CPU, GPU, active cooling, and six air vents. Unfortunately, since the processor is new for this console, we won’t know how well it performs until getting our hands on one. But, Snapdragon reckons it’s two to three times faster than the 720G mobile chip found inside the Samsung A72 from 2021. Connectivity is front-and-centre in the Razer Edge, with support for Wi-Fi 6E as standard, and 5G support in a higher-tiered version. Powering this is a whopping 5000mAh battery, 8GB of RAM, and 128GB of storage. As you’d expect, there are also two speakers, a mic, and a webcam that you can use for multiplayer gaming. Razer’s new handheld gaming console comes in at $400 – which prices it quite a bit ahead of competitors like the Steam Deck and Logitech G Cloud. But, you’ll find higher specs on the Razer Edge, justifying the price for some. It’s available to reserve from Razer now with a $5 refundable deposit, but won’t be shipping until January. Daniel and Union County Sheriff Peter Corvelli, in partnership with the Union County Board of County Commissioners, launched a county-wide community initiative designed to assist in making Union County a safer place to live, work, and visit. Known as the Safe Place Initiative, this program represents an active partnership between local police departments, businesses, social organizations, and schools to further New Jersey Attorney General Matthew Platkin’s statewide efforts to combat bias, hate, and violence. Safe Place participants will display a decal designating their establishment a “safe place” for any individual to enter and call law enforcement should they feel unsafe. This initiative began in Seattle, Washington as an effort to reduce LGBTQ+ bias incidents and hate crimes but has grown exponentially since its inception, and includes all forms of hate and bias incidents, as well as student bullying. The news comes as the South Korean Ministry of Science and ICT, along with the National IT Industry Promotion Agency (NIPA) recently announced it would foster national metaverse and XR-related firms to enter global markets. The Government previously revealed its ‘New Metaverse Industry Leading Strategy’ in January, which aims to develop the country’s digital ecosystem amid challenges linked to competitiveness and investments. XR Today interviewed Mija Kim, Chief Executive of The Welcome, Ltd, a South Korean firm specialising in public institution training and hosting international conferences. The CEO spoke about her company’s role in facilitating the growth and expansion of South Korea’s metaverse tech firms and the nation’s prospects for developing an extended reality (XR) ecosystem. XR Today: How does The Welcome facilitate networking for businesses, industries, and leaders as South Korea collaborates on its metaverse plans? Mija Kim: K-metaverse 2022, a massive project being implemented by the Republic of Korea’s Ministry of Science, ICT and Future Planning along with the National IT Industry Promotion Agency, aims to promote globally competitive companies specializing in the Metaverse by selecting and providing customized support to 70 K-metaverse companies that have developed strong technological capabilities. The Welcome, which is in charge of this project, connects global buyers and investors by providing consulting for competency strengthening to the 70 Korean metaverse-related companies selected based on strict criteria among groups at the early, middle, and advanced stages. Consulting is provided in diverse areas that meet the needs of each company such as technological development, human resources matching, and investment attraction. Mija Kim, Chief Executive of The Welcome To expand our overseas network, The Welcome sponsors global events such as the Augmented World Expo (AWE), Mobile World Congress (MWC) and XR FAIR by presenting exhibitions in pavilions and actively supporting participation in such events based on the characteristics of each company. A total of 70 companies participate in about 100 domestic and overseas events, generating valuable business performance through investment contact with buyers. The Welcome has enabled the convenient view of domestic company information, not only offline, but through an online business platform and a metaverse platform made by a Korean company, ZEP. XR Today: Which South Korean companies are the most active in building the Metaverse, and with which technologies? Mija Kim: We have divided companies into three stages according to their level of corporate growth. Among them, ten leading companies have the greatest global growth potential, where each of them is globally competitive and technologically capable. Notably, 3i corporation, a platform business company that connects smart pod hardware with applications, and NP, an emerging technology metaverse content company equipped with Korea’s largest XR stage, will make themed presentations at this year’s AWE. XR Today: Why do K-metaverse companies plan to expand globally, and how do they plan to compete in international markets? How do you select them for expansion? Mija Kim: Korean Internet platform companies independently pursue metaverse ecosystems which leverage emerging technologies such as artificial intelligence (AI) and the blockchain. Currently, they are forming a cooperative ecosystem in Korea through active collaboration, investment and acquisitions among IT companies and entertainment companies. Korean Wave-based intellectual property (IP) rights and a strong fan base are the basis of metaverse businesses to diversify profit models and enable global market entry. Domestic small and medium enterprises (SMEs) and startups also have the technology and ideas in each of their respective areas, but are facing difficulty in turning new platforms into businesses due to adverse conditions. Government assistance in projects like K-metaverse now seeks collaborative partnerships with global companies and attracts investment and market entry opportunities. This will provide support so that high-quality metaverse companies can help form South Korea’s future metaverse ecosystem. XR Today: What efforts has the Korean government made to build South Korea’s metaverse ecosystem? Mija Kim: The Korean government recently announced the “Implementation Plan for the Korean New Deal 2.0.” Its main objective is to promote new industries related to hyperconnectivity technologies. Additionally, it has established a “Development Strategy for a Virtual Convergence Economy” to pursue policies to boost XR usage, establish leading XR infrastructure, reorganise systems, and back corporate competitiveness. It will also increase investment and expansion of core technologies linked to the Metaverse such as XR, AI, the blockchain, network infrastructure, data, and digital twins.

Still, they often take second billing to contests for the Senate and the House, where the majorities stand a chance of switching practically every election cycle. But in 2022, gubernatorial races are not wanting for attention. Gubernatorial contests are among the highest profile of the midterm election roster. Like races for attorney general, secretary of state, and state supreme court seats, governors will play a vital role in the logistics of the 2024 presidential race. Governors’ signatures are required to send Electoral College vote tallies to Congress after a presidential election. It’s usually a formality, but in 2021, it became a flashpoint that led to violence at the Capitol on Jan. 6, when supporters of defeated President Donald Trump tried to stop certification by Congress of then-President-elect Joe Biden’s 306-232 Electoral College victory. Abortion also looms prominently in gubernatorial races. The Supreme Court’s June 24 Dobbs v. Jackson Women’s Health Organization decision, effectively making abortion law a state concern, means there could be dramatically different policies in place on the longtime hot-button social issue depending on who gets elected governor. In Pennsylvania, for instance, Democratic nominee Josh Shapiro, his state’s attorney general, has made support for abortion rights a central theme of his race against Republican rival Doug Mastriano, a state senator who has said the procedure should be illegal in virtually all instances. “After the Dobbs decision, abortion has become a huge issue across the country. Governors are the last line of defense and the first line of progress. We’ve seen in races across the country, including Michigan, Arizona, and Wisconsin, Republicans have taken extreme stances that are out of step with the electorate,” said David Turner, communications director for the Democratic Governors Association, told the Washington Examiner. Republican governors, meanwhile, tout their work combating the Biden agenda amid the president’s lackluster approval rating and political headwinds facing Democrats in the midterm elections, in which the out-of-power party usually picks up seats. That’s bad news for Democrats since the party holds the White House and majorities in the Senate and House, though by razor-thin margins in both chambers. “Whether you’re a parent seeing your grocery bill skyrocket or a young family struggling to keep up with the costs of a baby or a senior who’s seen your savings deplete, inflation and Democrats’ failures on the economy are front of mind for every voter,” Joanna Rodriguez, a Republican Governors Association spokeswoman, told the Washington Examiner. “And instead of working to address the problem sincerely, Democrats have decided to just pour gasoline on the fire with more spending and higher taxes.” Democratic governors aren’t ceding the mantle of fiscal conservatism. “They’ve been able to talk about their records of cutting costs for families,” said Turner, of the Democratic Governors Association. That includes Connecticut Democratic Gov. Ned Lamont cutting property taxes in his state and Michigan Democratic Gov. Gretchen Whitmer enacting car insurance rebates. Whitmer, elected governor in 2018, in May announced that $400 auto refund checks have been sent to nearly three-quarters of eligible Michiganders from the Michigan Catastrophic Claims Association catastrophic fund surplus. Republicans are also looking south as Nov. 8 nears, with a simmering U.S.-Mexico border crisis making life difficult for Democratic gubernatorial candidates politically. “Resources at every level of government are being drained because of the Biden administration’s failures at the southern border, but it’s not just border states paying the price,” said the RGA’s Rodriguez. “The record amounts of fentanyl and drugs flowing over the border into communities across the country is unprecedented, and Republican governors refuse to stand by and watch Americans die because Joe Biden and Democrats in Washington refuse to act. Governors are utilizing state resources in new and innovative ways, they’ve offered to meet with the president, and even after he ignored them, the governors still put forward policy solutions they knew would relieve the situation.” Gubernatorial races also draw attention because they differ from federal races in issue set, campaign texture, spending rules, and other areas. It’s not uncommon for states that tilt to one party or the other to elect members of the rival camp as governor because the job is seen as a more technocratic, get-things-done role. Three of the deepest-blue Democratic states have Republican chief executives: Maryland Gov. Larry Hogan, Massachusetts Gov. Charlie Baker, and Vermont Gov. Phil Scott. Conversely, a stalwart Republican state in presidential and Senate races, Kansas, has a Democratic governor, Laura Kelly. And Louisiana, where Trump beat Biden 58%-40%, has a two-term Democratic governor, John Bel Edwards. In Kentucky, too, where Trump won over Biden by an even wider 62%-36%, Democratic Gov. Andy Beshear has defied his state’s GOP partisan lean. Overall, the gubernatorial map plays in Republicans’ favor, said Rodriguez. “It’s considered incredibly difficult to defeat incumbent governors, but right now, Americans are living with the repercussions of Democrats’ failures in Washington and at state capitols every day, so it’s easy to see why the most vulnerable incumbent governors are in fact all Democrats,” Rodriguez said. “Each of these candidates has failed their states on issues that not only resonate nationwide but also play a unique impact in the culture and quality of life in their states: from crime in Wisconsin, Nevada, Oregon, and New Mexico to education in Kansas and Maine. Voters in blue states are seeing the success and growth of red states and realizing the only thing holding back their own state is their Democrat governor.” Democratic strategists and consultants counter that they’re the ones on the offense heading into Nov. 8. And under the right circumstances, they have a chance at overturning Republicans’ 28-22 edge in governorships — or at least tie, with 25 Democratic governors and 25 Republicans. Here are five of the most competitive gubernatorial races in 2022. ARIZONA. The governor’s office is coming open because Republican Doug Ducey is retiring due to state term limits. Kari Lake, who was a television news anchor on Phoenix station KSAV for 22 years, won a contentious Republican primary to nab the GOP nomination. Lake is a vocal proponent of Trump’s claims that the 2020 election was rigged despite no evidence surfacing to that effect. The Democratic nominee is Secretary of State Katie Hobbs, a former state senator and social worker. Hobbs is running as a nonpartisan problem solver who is willing to cross the aisle. Still, her role as secretary of state, which oversees elections, has become a focal point of the campaign. Multiple reviews have shown a clean and sound 2020 election in Arizona, where Biden beat Trump by less than 11,000 votes. Lake and state Republicans don’t accept those findings. That makes the race unusual in that a key area is less about the current gubernatorial contest and more about looking back to the events of late 2020. KANSAS. Democratic Gov. Laura Kelly is seeking reelection in what is otherwise a strongly Republican state. Kansas has voted for GOP presidential nominees from 1964 on and hasn’t sent a Democrat to the Senate since the 1932 elections. Kelly casts herself as a nonideological pragmatist. “As governor, Laura Kelly fulfilled her promise to be a moderate leader who would put Kansas back on track after years of fiscal mismanagement, harmful budget deficits, and underfunded schools,” Kelly’s campaign website says. Kelly’s Republican opponent is Kansas Attorney General Derek Schmidt, a former state senator and a congressional aide. Schmidt also is steering toward the middle ahead of the Nov. 8 election. Schmidt has said he respects the Aug. 2 Kansas vote on abortion, which decisively rejected a proposed amendment to the state constitution to allow the Republican-controlled legislature to restrict or ban the procedure. Kelly is the only Democratic governor running for reelection in 2022 from a state carried by Trump in 2020. Many Republicans still anticipate that frustrations with high inflation and red-state opposition to Biden will boost Schmidt’s chances of winning in November. NEVADA. Democratic Gov. Steve Sisolak is betting Silver State voters like his brand of business-friendly progressivism. The governor’s race is one of several competitive Nevada contests this year, along with Democratic Sen. Catherine Cortez Masto’s reelection bid and three of the state’s four House races, where Democratic incumbents are trying to hold on. Sisolak has earned the endorsement of the Vegas Chamber, a powerful business group in Las Vegas. Sisolak faces Republican nominee Joe Lombardo, who is Clark County sheriff. Lombardo in his gubernatorial campaign is leaning into education policy. Lombardo has thrashed Sisolak on education, criticizing him on school safety, curriculum claims, and pandemic-era school closures while pushing for “parents’ rights.” OREGON. The usually blue Pacific coast state, which hasn’t elected a Republican governor since 1982, has a three-way race on its hands. On her first day exploring Benin, Rachel asked a passerby for directions. Two weeks later, Rachel and the stranger were engaged. Within six months, they were married. Rachel grew up in Ottawa, Canada, the daughter of Haitian parents who’d immigrated to Canada in the late 1960s. As an adult, Rachel relocated to Washington DC for college, later working for a bipartisan tech program associated with the United States congress. Rachel loved this job, she loved the diversity of Washington and loved working in public service. When her US visa was up for renewal, Rachel, then in her early 40s, figured she’d work remotely for a few months before returning to DC. But rather than working from Canada, she hatched a plan to set up her desk further afield. Earlier that year, Rachel had submitted her DNA to an online ancestry site. Rachel had long known she was the descendent of enslaved Africans, but until she got the results, she hadn’t known where her forebears had lived. Now, she had a list of countries where she had roots: Senegal, Ivory Coast, Togo, Ghana and Benin. “DNA tests for a descendant of enslaved Africans has very deep significance for us,” Rachel tells CNN Travel. “Even though it’s not a precise science, when you get the map of where your ancestors came from, it’s an emotional journey.” Rachel arrived in Benin towards the end of her five month remote working trip. She’d already visited the other countries on her list, and her African trip was shaping up to be an extraordinary journey of self-discovery. Nevertheless, Rachel didn’t know what to expect from Benin. “Honestly, I don’t know if I could find Benin Republic on a map before this,” she says. She booked a room in a bed and breakfast in the port city of Cotonou, planning to stay there for two weeks — working from the B&B and exploring the country in her spare time. Following a couple of days settling in, Rachel ventured out for the first time. She planned to visit Ouidah, once one of the most active slave trading ports in Africa. She expected this would be a moving and thought-provoking experience. “I’m sure that one of my ancestors passed by there, just because of my DNA test,” says Rachel. Exiting her room, Rachel searched around for the manager of her bed and breakfast — she was looking for guidance on how best to travel to Ouidah. “She’s nowhere to be found. And then I look for the security guard, and the security guard is on break.” Rachel figured her next best bet was asking a passerby outside, so she opened the gates and glanced around. The first person she spotted was a man about to get on a motorcycle, parked just outside. Rachel greeted the stranger in French — as a French Canadian, French is her first language and it’s also the official language of Benin — and politely asked him how to get to Ouidah. “You have to go to a certain intersection downtown, where all the bush taxis are,” explained the stranger. “You find the taxi going to your destination, you pay for your seat, and then you’ll get there.” He started passing on directions to the intersection, but then, realizing they were a bit complicated, changed his tune. “If you want. I can bring you there, it’s about 10 minutes away,” he suggested, gesturing to his bike. It was about 9 a.m. Rachel was wary of trusting someone she didn’t know, but she decided she was unlikely to come to harm in broad daylight. She agreed. “I take a chance, hop on the back of his motorcycle, no helmet,” she recalls. The motorbike-riding stranger was Honoré Orogbo, a single father and business owner in his thirties who’d lived in Cotonou all his life and just happened to be passing by that morning. When Rachel opened the bed and breakfast door, Honoré had just finished eating some breakfast he’d grabbed from a nearby street kiosk. From the outside, Rachel’s accommodation wasn’t obviously a B&B. Honoré says he assumed she was the owner of the house. It was only when she asked for directions that Honoré realized Rachel was a visitor. When Rachel and Honoré arrived at the taxi rank in Cotonou city center, they realized the one heading to Ouidah was pretty empty. Honoré explained it would be some time before it departed — the driver wouldn’t leave until the taxi was full. Rachel was disheartened. She didn’t have time to wait around — she wanted to spend the whole day in Ouidah without feeling rushed, and to safely return to Cotonou before sundown. Sensing her disappointment, Honoré came up with a suggestion. He had a friend in Ouidah he’d been hoping to visit — while he hadn’t been planning to go that day, he could, he had a day off. “I’m like ‘Cool. I’ll pay for gas. Let’s go,” recalls Rachel. Just over an hour later, they arrived in Ouidah. “He shows me how to get back — where the bush taxes are that I can get back that afternoon — and he shows me where the Slave Museum is. And I’m like, ‘Okay, good to go. Thanks, sir,” recalls Rachel. But before they were due to go their separate ways, Rachel asked Honoré if he wanted to get brunch. She wanted a bite to eat before she started her tour — and extending the invite to Honoré felt like the polite choice, he’d gone out of his way to help her, after all. Honoré agreed, touched by the gesture. The two sat down to eat. Rachel was aware that she was a woman traveling alone, and while Honoré had been nothing but polite and respectful, he was still a stranger, so she told him she was married. She also didn’t share details of her job, or her life in the US. But she did explain how she was hoping to travel around Benin over the coming days. She asked Honoré if he had any friends or contacts who worked as chauffeurs or tour guides, and who might be interested in escorting her around over the next couple of days. She figured that might be easier than relying on taxis. Honoré contacted a tour guide friend, but he was fully booked “So I said, ‘Well, how about you? Can you be my escort? You helped me out this morning, can I just pay you to do that for three days?” recalls Rachel. “No, I’m not a I’m not a tour guide,” said Honoré. “I don’t know my country’s history by heart, and that’s not what I do.” Rachel backtracked. She didn’t really need a tour guide — there would be experts at all the historical sites she planned to visit — she just needed a ride. After a bit of back and forth, Honoré agreed to drive Rachel. “When she insisted, I said ‘Why not?” Honoré recalls today. He wanted to help Rachel, Honoré says. She seemed like a “good person,” based on the way she’d approached him, the way she’d asked him questions and the way she’d invited him to brunch. The two agreed Honoré would drive Rachel around for the next few days, starting that day in Ouidah, and Rachel would pay him for his services. For the rest of the week, Honoré took Rachel to Benin’s most important sites. Touring Benin was a powerful experience for Rachel. She says visiting the slave fort, inside Ouidah’s Museum of History, “is a pilgrimage that every afro-descendant should visit to remind us of the cruelty that our ancestors survived.” “I didn’t know this before going there in person, but if Las Vegas was taking bets on the survival of enslaved Africans, the odds of my being alive today would have been slim to none,” says Rachel. “I am a walking, talking miracle. I am the ‘one percent.’ I owe it to those who didn’t make it to live my best life.” While traveling around Benin, Rachel and Honoré talked. While Rachel still didn’t disclose many details about her personal circumstances, but she found herself opening up to Honoré about her thoughts and feelings. Honoré opened up in turn. “First conversations were about learning about myself, my family, my situation, who I am, who I really am,” he says. “We were very open and very candid, because we were strangers and we’ll never see each other again,” recalls Rachel. She remembers being touched when Honoré explained that he didn’t have a new model of motorcycle because he put all his money towards his son’s education. “He says ‘I’d rather have my kid have those opportunities than drive a fancy motorcycle.’ And I thought, ‘Wow, those are the values of my parents.’ I saw myself in those values,” says Rachel. In one of their many conversations, Honoré mentioned his brother was a tailor. On their fourth day together, Honoré took Rachel to a market to help her buy fabric that his brother could make into a dress. Rachel was overwhelmed by the choice — so much so that she asked Honoré to pick his favorites. He opted for two pieces of colorful, bright Ankara fabric. The third option was a white, gray, lace style, called lessi. Rachel loved it, and figured the resulting dress could be “appropriate for a baptism or some kind of special occasion.” In one of their many conversations driving to Benin landmarks, Honoré mentioned to Rachel that he would usually travel to Lomé, the capital of the neighboring country of Togo, when he and his friends wanted a night out. “I can’t guarantee that I’ll ever come back here. This is a once in a lifetime trip where I’m getting paid while I’m working in a foreign country. I want to take advantage of every opportunity,” she remembers thinking. “So I said, ‘Well, I have to go back to work this week. But next weekend, if you’re willing, I could get two hotel rooms and we could go to Togo together.” The following weekend, Honoré took Rachel to a poetry slam night in Lomé, followed by a bar with live music. These risk behaviours tend to cluster within an individual, which could have detrimental health effects. In this study, we aimed to examine the clustering effect of lifestyle risk behaviours on cardiovascular disease (CVD) and CVD risk among adults in the United Kingdom (UK). We performed a latent class (LC) analysis with distal outcomes using the UK Biobank baseline (2006–2010) data. First, we estimated LC measurement models, followed by an auxiliary model conditional on LC variables. We reported continuous (mean difference—MD) and binary (odds ratio—OR) outcomes with 95% confidence intervals. We included 283,172 and 174,030 UK adults who had data on CVD and CVD risk, respectively. Multiple lifestyle risk behaviour clustering (physically inactive, poor fruit & vegetable intake, high alcohol intake, and prolonged sitting) had a 3.29 mean increase in CVD risk compared to high alcohol intake. In addition, adults with three risk behaviours (physically inactive, poor fruit & vegetable intake, and high alcohol intake) had 25.18 higher odds of having CVD than those with two risk behaviours (physically inactive, and poor fruit and vegetable intake). Social deprivation, gender and age were also associated with CVD. Individuals’ LC membership with two or more lifestyle risk behaviours negatively affects CVD. Interventions targeting multiple lifestyle behaviours and social circumstances should be prioritized to reduce the CVD burden. Introduction Noncommunicable diseases (NCDs) are responsible for 71% of all deaths worldwide1. Lifestyle risk behaviours such as smoking, physical inactivity, alcohol consumption, and unhealthy diet increase the risk of dying from NCD1. Cardiovascular diseases (CVDs)1,2,3 account for most NCD deaths globally. In the UK, 11% of the population live with CVD, and CVD accounts for 25% of all deaths4. In 2010, the American Heart Association (AHA) recommended seven cardiovascular health behaviours (Life’s Simple 7) to reduce CVD morbidity and mortality in the general population, including smoking, diet, physical activity, body mass index, blood pressure, total cholesterol, and fasting glucose5. The 2019 AHA guideline on the primary prevention of CVD recommends that people engage in a healthy lifestyle throughout their life6. Lifestyle risk behaviours, such as smoking, alcohol consumption, poor diet and physical inactivity are major risk factors for the development of CVD7,8. Physical inactivity and increased sedentary time are important modifiable risk factors for cardiometabolic disease9,10. Physical inactivity is also the fourth leading cause of disease and disability in the UK11. Sleeping too much or too little is also strongly associated with CVD12,13,14. People engaging in multiple risk behaviours tend to have poor health outcomes compared to those engaging in one risk behaviour15. Thus, identifying people with multiple risk behaviours provides insight into where policies should target to reduce inequalities in health16,17. While previous studies have investigated the clustering of lifestyle behaviours for people with CVD18,19 and the general population20, most did not include all major lifestyle behaviours, such as smoking, alcohol intake, fruit and vegetable consumption, physical activity, sitting behaviour, and sleep. To analyse the co-occurrence of multiple health-related behaviours, different statistical approaches have been proposed in the literature21. However, these approaches are mainly focused on identifying clustering of risk behaviours and not estimating their effect on a distal outcome. Latent class (LC) analysis with a distal outcome is important for identifying how different lifestyle risk behaviours occur together among participants based on indicator variables, and to estimate the effect of LC membership on a distal outcome22,23,24. The effects of LC membership (clustering of lifestyle risk behaviours) on CVD and the risk of developing CVD have not yet been investigated. This study aimed to examine the prevalence of six lifestyle risk behaviours (smoking, poor fruit and vegetable consumption, alcohol intake, physical inactivity, prolonged sitting, and poor sleep), and clustering patterns of these lifestyle risk behaviours. In addition, we aimed to identify and estimate the effect of socio-demographic characteristics (age, gender), Townsend deprivation index and LC membership on CVD, and CVD risk. Methods UK Biobank has ethics approval from the North West Multi-centre Research Ethics Committee25. According to this approval, researchers do not require separate ethics applications. All participants provided written informed consent. This study was carried out in accordance with relevant guidelines and regulations. Study population We analysed data from the UK Biobank study that included more than 500,000 middle-aged (38–73 years) adults recruited from 22 sites across England, Wales, and Scotland. We used baseline data collected between 2006 and 201026,27. Socio-demographic, lifestyle (smoking, alcohol consumption, dietary intake, physical activity, sitting time and sleep duration) and medical history were collected using the touchscreen questionnaire27. Disease categories The UK Biobank collected self-reported medical information, such as CVD based on physician diagnosis. To define participants’ CVD status, we used data on vascular/heart problems diagnosed by a doctor (Field ID = 6150). Under this Field ID, four CVDs were reported: heart attack, angina, stroke, and high blood pressure. For this analysis, participants who were reported to have at least one of these diseases were classified as having CVD, not otherwise. A total of 283,172 participants without missing data were included. For the 174,030 participants without CVD, we computed a 10-year CVD risk score28 using the Framingham risk score function from the CVrisk package29 in R30. The variables included in the 10-year CVD risk calculation were age, gender, total cholesterol, HDL cholesterol, systolic blood pressure, blood pressure medication, smoking and diabetes status28. Lifestyle behaviours This analysis used six lifestyle behaviours: smoking, physical activity, fruit and vegetable consumption, alcohol intake, screen and driving time, and sleep duration. Physical activity The UK Biobank collected data on physical activity using adapted questions from the short International Physical Activity Questionnaire (IPAQ)31 that includes the frequency, intensity and duration of walking, moderate and vigorous activity. UK Biobank data on time spent on moderate and vigorous activity was added and converted to a metabolic equivalent of task (MET) score. Participants were classified as active if they had ≥ 750 MET min/week or inactive (� 750 MET min/week), based on the 2019 AHA guideline6. Fruit and vegetable intake The UK Biobank collected data on dietary intake using the Food Frequency Questionnaire32. The NHS guideline recommends every individual to eat at least 5 portions of a variety of fruit and vegetables every day33,34. Data on fresh fruit (pieces), dried fruit (pieces), salad/raw vegetable (heaped tablespoons) and cooked vegetable (heaped tablespoons) were combined to calculate portions. Participants consuming at least 5 portions of fruits and vegetables per day were considered to have adequate fruit and vegetable intake. Alcohol intake Participants were asked for the number of pints of beer, glasses of wine, and measures of spirit consumed in the last week. Since alcoholic drinks differ in the amount of alcohol content, we converted each drink into equivalent standard units (1 unit contains 10 ml of ethyl alcohol)35. We calculated total weekly units of alcohol consumption by adding the units of beer, wine, and spirits. Based on the NHS guidelines35, we grouped participants as low-risk drinkers (≤ 14 units per week) or high-risk drinkers ( 14 units per week). Smoking To measure smoking status, participants were asked, “Do you smoke tobacco now?”. Response options were “Yes, on most or all days”, “Only occasionally” and “No”. Those who responded “yes” or “smoke occasionally” were coded as 1, current smoker, while those who responded as “no” were coded as 0, not a current smoker. Prolonged sitting Total sitting time was calculated from the sum of self-reported hours spent watching television, using the computer, and driving during a typical day. Based on the estimated total sitting time, participants were categorized as low risk sitting (� 8 h/day) or prolonged sitting (≥ 8 h/day)36,37. This was based on the evidence of greater mortality risk for each increased sitting time category compared with � 8 h/day36,37. Sleep duration To measure sleep duration, the UK Biobank asked participants ‘About how many hours sleep do you get in every 24 h? (please include naps)’. Sleep duration was split using predefined thresholds from the literature; � 7 h, 7–8 h and  8 h13. Based on these cut points, participants were grouped as having ‘poor sleep’ (� 7 or  8 h/night) and ‘good sleep’ (7–8 h/night). Socio-demographic variables Socio-demographic characteristics (age and gender), and Townsend deprivation index (TSDI) were included in the latent class analysis (LCA) model. I’ll take it!   OK, we have some news,  so let’s get into it, shall we? An Anne Arundel County Police officer has had his police powers suspended after he was arrested in Baltimore for second-degree assault. Because this is Baltimore City, we do not have much information. Still, we learned from County police that they became aware of the arrest on Thursday and that Officer T. Thomas, who is assigned to the community services bureau, has had police powers suspended and has been reassigned to an administrative capacity while this all gets resolved. Officer Thomas is a 2-year veteran of the force. A teacher was hit by a car during the morning drop-off at the Monarch Academy on Capital Drive in Annapolis on Friday. A parent dropped off their child when they accidentally struck the teacher assisting with the process. She was taken to Shock Trauma and released later on in the day. Obviously, the school sent home a letter advising parents to use extra caution while dropping off or picking up and said they would have counselors available to anyone that needs to speak to one. And speaking of schools. Fingers crossed this morning for Dr. Bedell’s partial solution to the bus shortage situation. Somehow this tube of toothpaste is being squeezed, and they have found a way to serve 37 bus routes that had not been served before without new buses or new drivers. Things can change, of course, so please check aacps.org/buses for the latest. And if Dr. Bedell legitimately was able to find 37 routes in the first six weeks of school that the entire transportation department could not find for two years–one has to wonder how fouled up it really is. Some silly election news. A group aligned with the Dan Cox Campaign, calling themselves the Maryland Voter Integrity Group, says they will want to examine all ballots and do their own manual count to ensure the machines are reading votes correctly and there are no hidden algorithms. Governor Hogan and Democratic Candidate Wes Moore have said they have the utmost confidence in the machines and system. Meanwhile, Republican Dan Cox has refused to say he would accept the results of the election More election news…and this is purely to make sure you are informed. Tomorrow at 7 pm at Maryland Hall, there will be a County Executive Debate by The Capital and the Anne Arundel Chamber of Commerce. It’s free, but they would like you to register, and you can do it at marylandhall.org. And on Wednesday, up in the Dena at the Our Lady of the Chesapeake Church on Ventnor Road, the Greater Pasadena Council will host a candidate forum for Congressional District 3, District 31 for both Delegate and State Senator, County Executive, and Sheriff. It gets underway at 7 pm and will last for 2 hours. Sports news. Let’s just not. Suffice it to say that the Ravens and the Midshipmen figured out ways to snatch defeat from the hands of victory. The conference is scheduled to be held between October 31 and November 1, hosting a number of debates between local government officials, executives, and industry observers, as indicated by the event’s agenda.  The conference is expected to accommodate more than 500 representatives of financial institutions from across the world, and more than 200 speakers from China and overseas, according to Hui.  The latest announcement comes as cryptocurrency investors are increasingly worried about Hong Kong’s regulatory ambiguity on crypto. The city’s lawmakers are advancing plans to require licensing for crypto trading platforms through an amendment to Hong Kong’s anti-money laundering legislation. Such a change would require companies to offer such services exclusively to professional investors with a portfolio of at least HK$8 million (US$1 million). Should it go through, the decision could discourage numerous crypto investors from carrying out their business in Hong Kong.

Yet, there’s more coming, and from an unlikely source: a rapidly deteriorating relationship between the US and Saudi Arabia. Earlier this month, Riyadh defied Washington, leading the OPEC+ cartel, which includes Russia, to cut oil production. The move has put a floor on oil prices, which have stabilized between $90 and $100 a barrel. As a result, inflation is likely to be more persistent than previously expected, probably forcing central banks into further monetary tightening, which would bite bond investors. This enraged the US.  Now American-Saudi relations are at their worst than since the assassination of journalist Jamal Khashoggi in October 2018. Since Riyadh agreed to cut oil output, the kingdom and the White House have engaged in a war of words. The Saudi Foreign Affairs Ministry released a statement saying that Washington wanted OPEC+ to delay the cut by a month — implying that the problem was really the US midterm elections. The White House responded accusing Riyadh of “spinning” excuses.  The key for what comes next is NOPEC. This is the “No Oil Producing and Exporting Cartels” Act, a bill that proposes subjecting OPEC to the Sherman antitrust law that was used more than a century ago to break up the oil empire of John Rockefeller. If enacted, the White House would be able to sue Saudi Arabia and its allies, currently protected by sovereign immunity, for manipulating the global oil market. There’s a real possibility the bill may see the light of day. But despite how much Biden wants to punish Saudi Arabia and OPEC for cutting oil supply, it would be far wiser to avoid escalating the legislation. Otherwise, the risk is that Saudis dump US financial assets, redirect oil sales and openly talk about pricing oil in other currencies.  For the last 25 years, NOPEC has been a staple of Washington — always a threat but never a law. President after president, whether Republican and Democrat, have argued against passing it. But Joe Biden, who once supported a similar bill as Senator, has said he’s ready to work with Congress to curb OPEC influence. If NOPEC was to become law — a big if — OPEC nations may retaliate by dumping some of their financial holdings in America. This means that NOPEC could come at a big price for the US.  As of the end of July, Saudi Arabia, the United Arab Emirates, Kuwait and Iraq altogether held, directly, about $246 billion in US Treasuries, according to government data. The real number is likely to be higher, as Middle East nations also hold bonds via tax havens such as Luxembourg, the Cayman Islands, Bermuda, Switzerland and Ireland. Although their cache is unlikely to be higher than 5%-10% of total foreign holdings of American sovereign debt — and they are likely to be significantly lower than the $970 billion held by China — dumping those assets will rock an already jittery Treasuries market. “There is a real risk that this diplomatic dispute could intensify,” said Helima Croft, an oil analyst at RBC Capital Markets who’s well connected in Washington and Riyadh. “We would not be surprised to see suggestions in the coming days that Gulf countries could liquidate their US financial holdings if NOPEC becomes law.” From what I hear from officials in the Middle East, the suggestions are already coming loud and clear. Are those empty threats? Maybe. After all, OPEC nations would likely suffer losses if they were to offload the assets and have few other options to park their money. But does anyone in Washington really want to test them? NOPEC has long been considered the nuclear option. No one has answered what would happen next if the bill was passed. Would the US government ask for an antitrust investigation into OPEC? Would it actually go as far as suing the Saudis in federal court? And if a lawsuit is filed and the US does win, can it enforce any compensation? Would it be worth the potential retaliation?  The White House needs to think about those questions — and whether it really wants to answer them. Senator Chuck Grassley, a Republican from Iowa, has now attached NOPEC as an amendment to the annual Pentagon spending bill, giving it a serious chance of getting a vote on the floor of the Senate next month. It’s unclear if the amendment has the votes. But the last time the bill came this close to passing was in 2007, when it got approved by the House of Representatives in a 345-72 vote and the Senate by 70-23, only to die after George W. Bush threatened a veto. Biden must finally decide where he stands. In 2000, when oil prices were rising, he co-wrote a letter as senator to then-President Bill Clinton urging the White House to sue OPEC either in US federal court or at the International Court of Justice in the Hague. In reality, there are a lot more working opportunities than you might think, from jobs in management, event planning, operations, social media management, and marketing just to name a few. Today, we will be talking with Jack J, a professional Data Scientist who is applying AI to competitive gaming and esports. This marks the start of a series about those jobs that may be unnoticed with the possibility of learning from those who already have worked in the world of esports. Interview with Jack J, the founder of iTero Gaming Q: Hi Jack J and welcome! Can you kindly introduce yourself and walk us through how you got into the esports world?  Sure, I’m Jack J and I’m a professional Data Scientist, formerly of HSBC and Deloitte, and now the founder and CEO of iTero Gaming, a start-up focusing on applying artificial intelligence (AI) to competitive gaming. I’ve been casually involved in esports for years. Everything started off with me using the skill set that I was developing as part of my day job as a Data Scientist and applying it to gaming-related data. I worked on the analysis of teams and players’ performance, figuring out which dragon souls were the strongest and writing various articles about each meta and what were the most optimal champions. Over time I became more and more involved and in 2021 I was invited to visit the Excel Esports gaming suite in Twickenham Stadium, London. There, I had the pleasure of meeting their analyst at the time (Logan ‘Owl’ Mansfield) who gave me a glimpse into the inner workings of an esports team. From that day onwards, I knew I wanted to explore my idea more fully, so I handed in my notice at work and founded “iTero Gaming” with a rough plan of “using AI for gaming”. Q: So what is iTero Gaming? And what was the idea behind the project? Tell us more about how you brought AI into esports.  The original concept for iTero Gaming was to build an AI capable of improving the very best professional teams in the world. The idea is that although players and coaches have a considerable understanding of the game, there are also some blind spots. While a person can only watch a limited number of games each day, AI can absorb millions of data points and learn incredibly complex rules. That isn’t to say that I believe that AI will ever be in a position to replace a coach, but it certainly can improve their understanding by identifying hard-to-spot patterns. Throughout the 2022 LEC Spring Split, I had the chance to work with various LEC coaches and analysts to better understand how the draft impacts a team’s chance of winning the game, with the intention of creating a product capable of giving teams the edge in Champion Select. Ultimately, we ended up deciding to take those same technologies that we built specifically for esports and made them available to everyone through the iTero Drafting Coach. Q: What are your and iTero Gaming’s goals for the next few years? This year saw the launch of our first publicly available tool, the drafting coach, which works for any draft-based queue (it doesn’t support ARAM yet, sorry!). So far we’ve had fantastic feedback, as well as plenty of suggestions for improvements, so we’ll continue to focus on perfecting our tool and aim to become th best in the market. This includes adding AI-powered item and rune recommendations, as well as some extra functionality for creating your team’s perfect Clash composition. As for next year, we will move on to developing the next phase of our project. This is going to be a more holistic League of Legends AI coach, capable of analyzing not just your decisions in draft but your behaviors throughout the game. There are also plans to explore how a tool like this could be used in other competitive games, like Valorant or CS:GO. Finally, we have ambitions to return to our roots of esports, and we have plans to become more involved in that space by making full use of the advanced technologies we have available. More on that one another time…! Q: For those who aren’t familiar with it, why do you think data analysis is important for esports and League specifically? Unlocking data in the right way will create a fundamental shift in esports. There’s plenty of stories out there that show this in more traditional sports. Look at the “Moneyball” story in baseball, how mathematical models changed the rhythm of baseketball, or how Brentford football club changed their fate by using data to pick out the best players. Essentially, data analysis is the natural progression of any sport and provides the next phase of innovation once getting ahead based on raw talent alone is no longer possible. For example, in League, the meta can become stale very quickly. Champions are repeated over and over again because teams have no systematic ways of testing various team compositions and counterpicks. In such a situation, data analysis comes into its own place with the goal of helping teams pinpoint the most effective team compositions and strategies. That being said, this process is time-consuming and due to the time constraints and how the game is constantly updated every two weeks or so, it can be tough to find what works best. With AI, instead, we can more effectively predict the results of a game based on the draft, and have the ability to reinvent the meta time and time again by providing those secret pocket picks hidden away in the data at a much faster pace. Q: You previously have worked on jung.gg, a project born to help players find the best jungle paths. Riot recently announced their intention of bringing a similar built-in tool. What’s your opinion about it and how do you think it will impact the player base overall? Will it help new players get into League? There exists an interesting relationship between app developers like myself and game publishers. We spend years building out tools that will add value to a player’s experience, and if those tools are well-liked and become mainstream then it’s great for us and great for the users. However, if it’s great for the players that manage to discover the tools, why not just add them into the game natively and offer that value to everyone? This is something we fundamentally agree with and think that Riot’s recent announcement to include jungle pathing recommendations is the right move. Now, of course, that means that jung.gg no longer offers the same value to users as it used to, so we made the decision this month to take the site down. This is the natural lifecycle of projects like these: the best thing we can do is to keep innovating and improving on our projects so that we’re always offering more value. As to this change specifically, I believe it still hits that balance of making the game easier to understand for new players without taking away the advantage of experience. In other words, a new player will be given the basic understanding of what a good jungle path looks like, but the seasoned player will still have the advantage of knowing how to adapt these paths to their specific circumstance. In that way, it’s very similar to the item recommendation system. However, I will also add that although I believe this to be a positive change, I do not think it will make any major difference in attracting and retaining new players. If Riot wants to solve that issue, they need to deal with smurfing, toxicity and bots – these issues are considerably more impactful to a new player’s experience than any amount of help with item, rune or jungle path recommendations. These are also areas that AI is in a prime position to help with. Q: You have worked part-time with teams and players before launching your own AI-based tool. What are some of the tips that you would like to give to newcomers that would like to get into esports analytics? Are there lots of chances to get into esports through this career path? I’ve recently started a series of interviews myself where I speak to esports analysts in the LEC or LCS and ask them this very same question. A consistent piece of advice they have all given is that the best thing for a newcomer to do is to start by creating something useful. It doesn’t need to be perfect, the technology doesn’t need to be sophisticated and it doesn’t need all the bells and whistles. Just create something, in any way that you can, that could offer value. Then, find people to give you feedback. Accept the fact that your first attempt will be your worst attempt. The great thing about getting into esports is that everyone is very approachable. The iTero Discord, for instance, has a handful of LEC and LCS coaching staff and I am confident every single one of them would be happy to provide feedback, as long as you’re proving to them that you’re trying to provide value. The only failure is not having tried. As to the number of opportunities in this area, being an analyst has one very important advantage: the skills are transferable. If your aim is to join the LCS or LEC as an analyst, then yes, it will be tough and there are only a limited number of roles available. However, as part of that journey, you’ll need to learn the technical skills required to get, examine and display data. These skills are incredibly valuable, both in and out of esports. For instance, you could look at roles as a business or marketing analyst for an esports team and use that as a way of training your skills and meeting the right people to eventually move into being a team analyst. Besides, esports is only growing and so the number of analyst roles will grow with it – there are very few reasons not to try!

Sterling gained 1% against the dollar to trade around $1.13 in morning trading. The news also saw the interest rate – or yield – on UK government bonds fall. The drop in yields suggests financial markets are welcoming the prospect of changes to economic plans. Monday is the first time the UK government bond market has reopened since the Bank of England’s emergency support programme ended on Friday. On Friday, Prime Minister Liz Truss sacked Kwasi Kwarteng as Chancellor and said the mini-budget “went further and faster than markets were expecting”. The mini-budget was blamed for causing turmoil in the financial markets. The aftermath of Mr Kwarteng’s announcements on 23 September saw the pound slump to a record low of $1.03 and the cost of government borrowing rise sharply. “The chancellor will make a statement later today, bringing forward measures from the Medium-Term Fiscal Plan that will support fiscal sustainability,” a Treasury spokesman said. Mr Hunt is expected to fast-track many billions of pounds worth of tax and spending measures from his debt plan, announcing them a fortnight earlier than expected. It is the latest of a series of U-turns on policies announced in the mini-budget. The announcement of the £18bn U-turn on corporation tax on Friday and the firing of Mr Kwarteng did not appear to reassure investors, with UK government borrowing costs climbing on Friday afternoon. Investors warned that whatever Mr Hunt announces will need to “add up”. “I think you’ll see a positive reaction to the statement, assuming that the math adds up a bit more than it did before,” Shanti Kelemen, chief investment officer at M&G Wealth, told the BBC. “What we saw on Friday, was we had markets rise in in the lead up to the news the news that Kwarteng was resigning, but then as soon as it happened, we had a sell-off afterwards. “So I think it’ll be important that the actual content of what’s being delivered adds up and has some more meat and numbers behind it than what we’ve seen previously.” The Bank of England stepped in to stabilise the financial markets following the mini-budget, announcing an emergency bond-buying scheme. Ms Kelemen said that the latest moves from the chancellor showed he acknowledged the government’s role in reassuring the markets. “They’ve recognised that the uncertainty is damaging the economy,” she said. “You also see the Bank of England won’t be supporting markets this week. So I think it shows the government is taking a bit more responsibility rather than relying on the Bank of England to buy all the debt.” The shift in the government’s economic policies and market turmoil in recent weeks has led to Goldman Sachs downgrading its forecasts for UK economic growth. The investment bank revised its 2023 UK economic output forecast from a 0.4% drop to a 1% contraction. Goldman said it expected a “more significant recession in the UK” in part due to “significantly tighter financial conditions” and the planned higher corporation tax rate from next April. Consultancy Pantheon Macroeconomics said the prime minister’s decision to appoint Mr Hunt as chancellor had “done little to shrink the risk premium embedded in UK assets”. “Households and businesses, therefore, are still facing a huge increase in their borrowing costs,” their analysts said. They added the forthcoming real-terms reduction in government spending looked “set to be bigger than in the 2010s”. Or an older woman, with elevated blood pressure, a former smoker perhaps. These stereotypes of the usual heart attack candidate might explain why spontaneous coronary artery dissection (SCAD) is often misdiagnosed or underdiagnosed, and why patients with SCAD feel so confused, anxious, misunderstood, and abandoned by the health system. Findings of our qualitative study of survivors of SCAD, published last month in the international journal PLOS ONE, outline these and other psychosocial impacts of this challenging condition. What is SCAD? SCAD is an increasingly recognised cause of acute coronary syndrome and sudden cardiac death in people without classic cardiac risk factors. Unlike acute myocardial infarction (AMI) due to atherosclerosis, SCAD-AMI is non-atherosclerotic and occurs when a coronary vessel develops a tear or haematoma causing coronary artery obstruction. This arterial obstruction results in heart attack, arrhythmias, or sudden death. SCAD is a relatively rare condition, accounting for 1-4% of cases of acute coronary syndrome overall. SCAD predominantly affects younger women, accounting for up to 35% of AMIs in women aged 50 years and under. It is the most common cause of pregnancy-related AMI, most often occurring in the peripartum period. SCAD is also associated with underlying systemic vasculopathies such as fibromuscular dysplasia, connective tissue disorders, migraine, and inflammatory disorders. Emotional stress and physical exertion often precipitate acute SCAD events. Hospital readmission and recurrence are more common after SCAD than following typical AMI. Due to its relative rarity and propensity to occur in younger healthy women, SCAD is commonly misdiagnosed or dismissed. Due to its sudden onset and high re-event rate, SCAD carries a large psychosocial burden, which is exacerbated by health professionals’ lack of awareness of the condition and its management. Psychosocial impacts of SCAD Our qualitative study involved 30 SCAD survivors, mostly women (n = 27), with an average age of 52 years, although some participants were in their early 30s. All had had their SCAD in the preceding 12 months. They participated in focus group discussions about how they coped during their SCAD diagnosis, hospitalisation and early treatment, and during the first year of recovery. “Lack of information” emerged as the overriding theme, highlighting the way in which health professionals’ lack of knowledge about and understanding of SCAD results in insufficient provision of information and support to SCAD survivors. Among the most challenging emotional impacts for SCAD survivors was “confusion and uncertainty” about the condition, including its diagnosis, causes, treatment options, guidelines regarding return to daily living and physical activity, prognosis, and risk of re-events. Emotionally, SCAD survivors felt anxiety, fear and vulnerability; loss and grief; isolation and loneliness; invalidation and abandonment; and frustration and depression. Lack of awareness and understanding of SCAD in the medical profession Study participants talked extensively about how their treating health professionals lacked clear knowledge and understanding of SCAD. This included those working in hospitals, as well as the cardiologists and GPs who managed them after discharge. As we noted in our article: “Participants stated that many health professionals ‘had never heard of SCAD’ and often ‘knew nothing about it’, and … that health professionals were not aware of differences between SCAD-related heart attack and typical atherosclerotic heart attack.” SCAD survivors were surprised and frustrated about this lack of clarity about SCAD: “The people in the emergency department (ED), even the ED physicians and senior nurses, they had not heard of it before. I was surprised about the knowledge deficits.” “There’s a lot of uncertainty, in what should happen, what shouldn’t happen, what is or isn’t the treatment. There seems to be no continuity … My GP didn’t even know what a SCAD was.” “The advice they give you is geared towards normal heart attacks, about improving lifestyle. The information is not targeted well at all.” Due to medical professionals’ knowledge deficits, SCAD survivors are often left feeling unsupported, invalidated and abandoned, and wondering whether they are receiving the best care possible: “The medical profession knows so little about it. So, when you have a SCAD, you’re not treated right. You want to know what happened, what to do next, and there’s no-one there to give you the information. You have to go and look for it yourself.” “My GP said: ‘what’s a SCAD?’ and I had to explain it to him. And he said: ‘Oh, it’s not a proper heart attack is it? You didn’t really have a heart attack, did you?’ You don’t feel like you’re getting the best care, or the best advice.” “It’s the feeling that the medical people who you’re talking to aren’t across what SCAD is and what its effects are. I’m not blaming them for that because obviously it hasn’t been greatly researched until recently, but it does add to the anxiety for sure.” Why is SCAD so difficult for patients? There are several features of SCAD that make it a particularly challenging and stressful condition for SCAD survivors. It occurs “out of the blue”. Most SCAD patients have few traditional cardiovascular disease (CVD) risk factors and few have any prior warning about the possibility of SCAD. They and their families are not prepared for what follows. It is relatively rare. SCAD accounts for only 1–4% of acute coronary syndrome cases overall and can be regarded as an uncommon condition. This means that SCAD survivors are isolated and lack opportunities for contact with and support from others in the same position. It is poorly understood. The lack of understanding of SCAD means survivors are left to navigate their recovery alone with relatively little professional support. They often receive contradictory advice from their treating health professionals. Its optimal management differs from that of traditional AMI. In general, typical medical treatments such as statins and interventions such as percutaneous coronary intervention are either not required or are contraindicated. In the absence of atherosclerosis and traditional CVD risk factors, SCAD is not suited to traditional CVD lifestyle management approaches. Despite this, “SCAD continues to be … managed as atherosclerotic acute coronary syndrome, which may harm patients with SCAD”. There is a lack of evidence for benefit or harm of physical activity. As physical exertion is a documented trigger for SCAD, there is a lack of clear guidelines for or limits to physical activity. This leaves many previously active SCAD survivors frustrated and confused about returning to previous levels of activity. It has a high recurrence rate. Major adverse cardiac events occur in 10–30% of SCAD survivors within 2–3 years, mostly due to recurrent SCAD-AMI. The recurrence rate reaches up to 37% by 5–7 years, and approximately 50% by 10 years. This high recurrence is of great concern to both patients and their health care providers.
