Ivermectin for Prevention and Treatment of COVID-19 Infectio... American Journal of Therapeutics
mortality, in secondary outcomes, and in chemoprophylaxis, among people with, or at high risk of, COVID-19 infection. Data sources: We searched bibliographic databases up to April 25, 2021. Two review authors sifted for studies, extracted data, and assessed risk of bias. Meta-analyses were conducted and certainty of the evidence was assessed using the GRADE approach and additionally in trial sequential analyses for mortality. Twenty-four randomized controlled trials involving 3406 participants met review inclusion. Therapeutic Advances: Meta-analysis of 15 trials found that ivermectin reduced risk of death compared with no ivermectin (average risk ratio 0.38, 95% confidence interval 0.19–0.73; n = 2438; I2 = 49%; moderate-certainty evidence). This result was confirmed in a trial sequential analysis using the same DerSimonian–Laird method that underpinned the unadjusted analysis. This was also robust against a trial sequential analysis using the Biggerstaff–Tweedie method. Low-certainty evidence found that ivermectin prophylaxis reduced COVID-19 infection by an average 86% (95% confidence interval 79%–91%). Secondary outcomes provided less certain evidence. Low-certainty evidence suggested that there may be no benefit with ivermectin for “need for mechanical ventilation,” whereas effect estimates for “improvement” and “deterioration” clearly favored ivermectin use. Severe adverse events were rare among treatment trials and evidence of no difference was assessed as low certainty. Evidence on other secondary outcomes was very low certainty. Conclusions: Moderate-certainty evidence finds that large reductions in COVID-19 deaths are possible using ivermectin. Using ivermectin early in the clinical course may reduce numbers progressing to severe disease. The apparent safety and low cost suggest that ivermectin is likely to have a significant impact on the SARS-CoV-2 pandemic globally. Click here to subscribe - https://www.glutenfreesociety.org/wxrn Children with autism are usually particularly selective eaters which can contribute to nutrient deficiencies. Thus, the risk for the nutritional deficiencies in children on restricted diets remains an area of scientific interest. In addition, several researchers have noted that patients with autism often have various metabolic and nutritional abnormalities including issues with sulfation, methylation, glutathione redox imbalances, oxidative stress, and mitochondrial dysfunction that can impact absorption of nutrients. These deficiencies can further exacerbate autism symptoms. Common deficiencies linked to autism include: Folate: Folate has a vital role during neural development. During pregnancy, large amounts of folate are required, and thus there is a substantial risk of deficiency . It has been known for decades that folate deficiency has been considered a substantial risk factor for the defects in the neural tube in the fetus. Vitamin B12: Vitamin B12 is an essential cofactor in methionine metabolism, and severe deficiency is associated with a significantly increased prevalence of both anemia and cognitive impairment. Due to often related digestive concerns B12 absorption can be impaired and has been found more abundantly in autism patients. Thiamine (Vitamin B1): Thiamine deficiency has been connected to many diseases of the central nervous system, including autism. Thiamine may be involved in autism through several factors including apoptosis (cell death), neurotransmission (signaling to/from the brain), and oxidative stress. Vitamin C: Vitamin C acts as an antioxidant, protecting the body against damage from free radicals. Oxidative stress markers such as lipid peroxidation have been found to be significantly higher in children with autism. Vitamin E: Similar to Vitamin C, vitamin E acts as an antioxidant and protects the body from oxidative stress, which is generally higher in children with autism. In three studies where vitamin E levels were measured, all data reported lower vitamin E levels in children with autism. Vitamin D: Just as vitamin D while in the womb and as an infant impacts autism risk (as discussed in the next section), recent studies have demonstrated that the level of vitamin D in children with autism is significantly lower than in children without autism. Vitamin A: It has been reported that vitamin A is capable of increasing the level of oxytocin in autism patients, and therefore, brain activity and social abilities significantly may be increased through oxytocin in the autism patients. Some studies have reported that 77.9% of children with autism suffer from vitamin A deficiency, and also that vitamin A deficiency exacerbates autism symptoms in patients because it plays an important role in numerous biological pathways of the central nervous system. Omega 3 fatty acids: Omega-3 fatty acids influence brain health and development in a number of ways, but as they are primarily found in fatty fish, which is not often a favorite food of many children, particularly those with limited diets, they are often not consumed. One meta-analysis of several studies suggested that supplementation of omega-3 fatty acids may improve hyperactivity, lethargy, and stereotypy in autism patients. But just because deficiencies are common doesn’t mean that children should blindly supplement. In fact, one study found that even in children who supplement, almost one-third remained deficient in vitamin D and up to 54% for calcium. This suggests that the issue may be absorption rather than intake. In addition, in many cases, supplementation led to excess vitamin A, folate, and zinc intake across the sample, plus vitamin C and copper among children aged 2 to 3 years, and manganese and copper for children aged 4 to 8 years. The takeaway? Don’t guess what deficiencies exist, get tested to know for sure so that you can target supplementation accordingly. And if deficiencies still exist after testing, you likely need to dig further into why those nutrients aren’t being properly absorbed. Iron deficiency anemia is one the most common nutritional deficits found in patients with celiac disease and non-celiac gluten sensitivity (NCGS), and it has been reported as the most frequent extraintestinal symptom in adult celiac disease. It may also be the primary symptom experienced by celiacs, even in the absence of diarrhea or weight loss. So you might be wondering, does gluten cause anemia? Patients with celiac disease or NCGS are often unable to properly absorb iron due to damage to the small intestine (known as villous atrophy). In addition, gluten can damage the acid producing cells of the stomach. Acid is a necessary component for iron absorption, so this lack of acid can lead to anemia even if the small intestine is not damaged (e.g., without villous atrophy). While eating gluten may not immediately cause the onset of anemia, eating gluten can cause iron loss over time. This initially leads to a slow or gradual onset of symptoms, but as more time passes, a major iron deficiency anemia can develop. As anemia leads to oxygen deficiency, it can reduce the body’s ability to generate energy and inhibit the healing process. This creates a difficult cycle for celiac patients to try to heal, especially if their anemia goes undiagnosed. Can a gluten-free diet help cure anemia? The next logical question is then, if iron deficiency is commonly caused by celiac disease or NCGS, can eating a gluten-free diet help resolve anemia in celiac or NCGS patients? One study of 190 adult patients with iron deficiency anemia studied this. The adults were screened for celiac disease by duodenal biopsy and the 26 people (24 women, 2 men) diagnosed with celiac disease were asked to follow a gluten-free diet alone without iron supplementation. After six months of a gluten-free diet, follow up duodenal biopsies and hematological tests were performed at 6, 12, and 24 months. At six months, 78% female patients recovered from anemia and 28% reversed their iron deficiency. At 12 months, all but one patient (94.4%) recovered from anemia and 50% of patients reversed their iron deficiency. After 24 months of diet, still only one patient was anemic, and 55.5% reversed their iron deficiency. Note this study did not include people with NCGS, but provides promising evidence that iron deficiency anemia can often be resolved in newly-diagnosed celiac disease patients by following a gluten-free diet.
