Estimation of tumor blood flow using Rb-82 dynamic PET (dPET) and perfusion CT (pCT) in the assessment of anti-angiogenic therapy: A pilot study | Journal of Nuclear Medicine
1375 Objectives: PET-CT has great potential in monitoring drugs pharmacokinetics and pharmacodynamics in anti-angiogenic therapy. We explore the feasibility of using dPET and pCT to assess tumor blood flow (TBF) in this context. Methods: Three patients with unresectable non small cell lung carcinoma (stage 3b) and colorectal carcinoma (stage 4) received 1 cycle of an anti-angiogenic drug therapy. Immediately before and 7-14 days after therapy, patients were injected with 50mCi of Rb-82 and imaged dynamically for 6 min using PET/CT. Ventricular input functions and TBF (ml/g/min) were computed using generalized factor analysis of dynamic sequences and 2-compartment kinetic modeling. pCT was also performed and TBF computed in contiguous slices (9.6mm tichk) centered on the lesion. Results: TBF measured with dPET and pCT followed the same trend: Pat1 had 26% reduction of TBF on dPET (10% reduction on pCT) and an unconfirmed partial response. In pat2, there was 15% reduction of TBF on dPET (38% on pCT) and the patient’s disease remained stable. In pat3, there was a TBF increase of 21% on PET (45% on pCT) and the patient did not respond to therapy. Our results show reasonable correlation between pCT and dPET perfusion estimates. Conclusions: dPET and pCT have an important potential role in early assessment of response to anti-angiogenic therapy. dPET and pCT derived tumor blood flows follow similar trends before and after therapy, with dPET having the advantage of estimating TBF within the entire tumor, rather than sampling a small region with pCT. Further studies are warranted. ### Learning objectives Coronary computed tomographic angiography (CTA) is now established as a clinically valuable non-invasive anatomical test for the detection and exclusion of significant coronary disease. A number of prospective multicentre trials have shown coronary CTA to be an ideal test for the exclusion and detection of coronary disease using invasive angiography as the reference.1–3 Despite this, owing to its relatively low positive predictive value of 48% and inability to determine functional significance,1 its use in international guidelines has generally been restricted to patients with chest pain at a low-intermediate risk of having coronary artery disease (CAD).4 For patients at an intermediate risk of CAD, functional testing is generally indicated, and for high-risk patients, invasive coronary angiography (ICA) remains the recommended diagnostic test. Although this strategy is designed to determine whether a patient’s symptoms are attributable to CAD, and specifically myocardial ischaemia, recent studies indicate that this approach has important flaws. In a study of almost 400 000 patients, Patel et al showed that up to 62% of the patients who underwent ICA in the USA were subsequently found to have no significant obstructive disease. Could procalcitonin (PCT) be a marker of effectiveness of antibiotic treatment (ABT) in pregnant with not severe community-acquired pneumonia (CAP)? | European Respiratory Society
There is no data on changes of PCT on the background of ABT in pregnant with not severe CAP. Aim: to study the role of dynamic of PCT in pregnant with not severe CAP as a marker of effective ABT. Materials: we studied PCT levels in 35 pregnant with CAP which formed the main group (age (Med [25–75%]) 25 [19–30] yrs, gestational age 30 [27–33] wks) and in a control group of 15 healthy pregnant (age 31 [25–37] yrs, gestational age 35 [34–37] wks). Methods: in pregnant with CAP PCT was measured on admission, before starting ABT (PCT-1) and on the 5th day of therapy (PCT-2). We used SMRT-CO scale to estimate the severity of CAP and respiratory rate (RR), body temperature (BT) and leukocyte count – to monitor clinical status. Results: on admission all patients had high BT (37,9 [37–38,2]°C ), increased RR (28 [24–31] per min.) and leukocytosis (11,9 [8,3–14,3]*109/l), also they had 2 or less points on the SMRT-CO scale which corresponds to not severe CAP. They were prescribed standard ABT with amoxicillin/clavulonate. On the 5th day of ABT we observed a significant decrease of BT, RR and leukocyte count. Dynamics of PCT is shown in [Table 1][1].
