Lurasidone hydrochloride (LH), the hydrochloride form of lurasidone with a charge-assisted bond N+H, is an atypical antipsychotropic agent for the treatment of schizophrenia. As a BCS class II drug, LH has a low oral bioavailability mainly due to its poor water solubility and low dissolution. In order to improve its solubility, amorphization of LH was performed and characterized. Unexpectedly, the dissolution rate of amorphous LH was much lower than that of crystalline LH. In addition, the amorphous LH powders quickly aggregated when contacting the dissolution media (water, 37°C), and formed a sticky gel adhering on the paddle. The follow-up polarized light microscope, XRPD, DSC, and FTIR analysis found that amorphous LH transformed to crystalline LH during dissolution. On the other hand, no such gelation phenomenon of amorphous lurasidone was observed under the same dissolution condition. However, the gel would reform when dropping concentrated hydrochloric acid slowly into the bottom of the medium during the dissolution of amorphous lurasidone, and XRPD/DSC/FTIR results indicated that the regenerated gel was consisted of crystalline LH, suggesting that the charge-assisted bond N+H in the structure of LH mediated the gel formation of amorphous LH during its dissolution process.

Objective:To investigate the antipsychotropic drug use among mental disorders in Shenzhen.Methods:130 mental disorders were investigated and assessed with the drug treatment.Results:50(31.85%)patients was used typical drugs,58(36.94%)patients was used atypical drugs,49(31.21%)patients was used Clozapine.64.17%(77)of the patients were treated with mono-pharmacy,jointly used 2 kinds was 33.33% (40),jointly used 3 kinds was 2.50%(3).Conclusion:The mental disorders can have a standardize drugs using in Shenzhen Baoan.The percentage of patients received new atypical antipsychotics is higher than the others.

Nephrotic Syndrome Caused by Immune-Mediated Acquired LCAT Deficiency | American Society of Nephrology
Lecithin-cholesterol acyltransferase (LCAT) is an enzyme involved in maintaining cholesterol homeostasis. In familial LCAT deficiency (FLD), abnormal lipid deposition causes renal injury and nephrotic syndrome, frequently progressing to ESRD. Here, we describe a 63-year-old Japanese woman with no family history of renal disease who presented with nephrotic syndrome. The laboratory data revealed an extremely low level of serum HDL and undetectable serum LCAT activity. Renal biopsy showed glomerular lipid deposition with prominent accumulation of foam cells, similar to the histologic findings of FLD. In addition, she had subepithelial electron-dense deposits compatible with membranous nephropathy, which are not typical of FLD. A mixing test and coimmunoprecipitation study demonstrated the presence of an inhibitory anti-LCAT antibody in the patient’s serum. Immunohistochemistry and immunofluorescence detected LCAT along parts of the glomerular capillary walls, suggesting that LCAT was an antigen responsible for the membranous nephropathy. Treatment with steroids resulted in complete remission of the nephrotic syndrome, normalization of serum LCAT activity and HDL level, and disappearance of foam cell accumulation in renal tissue. In summary, inhibitory anti-LCAT antibody can lead to glomerular lesions similar to those observed in FLD.

Objective. The purpose of the study is to compare the outcomes of patient activation before and after pharmacogenetic care in a coordinated care population. The process of pharmacogenetic care includes the patient’s acknowledgment of medication changes which are based on their genetic profile. This study will be framed by Dorothea Orem’s Self-Care Theory and Davies and Conley’s framework of genetic influences in prescribing anti psychotropic therapy. Background. Understanding patient’s behavioral reactions to pharmacogenetic care in terms of patient activation is important to understanding patient’s health outcomes. No published studies were identified in literature reviews conducted by this author which related to patient activation and pharmacogenetic care. There were no published studies found by the author using either, or both of, Dorothea Orem’s Self-Care Theory or Davies and Conley’s framework. Methods. This dissertation study is designed as part of a larger study nicknamed “PGXTIME”. The study is a longitudinal, utilizing one-group, repeated measure design. Participants will complete the Patient Activation Measure-Mental health (PAM-MH) before and after having pharmacogenetic testing (PGX). Medication recommendations will be provided to the primary care provider based on current medications and genetic testing results. Standard of care in South Dakota requires patient involvement and communications regarding genetic testing. This study requires a patient’s acknowledgment that their medications are remaining the same or being modified based on pharmacogenetic testing results. Additional information will be collected regarding the number of chronic conditions, the number of mental health conditions, the number of medications utilized, coordinated care tier level and number of genetic pathways affected. A theoretical model was developed based on literature review, Orem’s model, Davies and Conley’s framework, to understand direct and indirect variable relationships. Results. This study was stopped in January of 2016. Therefore, no study results are presented in this dissertation. Chapter one and two describe this study while chapter three describes the reason the study was stopped and how the study changed. Chapter four presents study results, and Chapter Five includes the discussion.
