Background One of the most common orthopedic problems is the incidence of pressure ulcer followed by immobility. This study aimed to investigate the effect of Aloe Vera gel on the prevention of pressure ulcer in patients hospitalized in the orthopedic ward. Method This study is a randomized, triple-blind clinical trial which was done on 80 purposefully selected patients in orthopedic ward in Arak town, Iran, 2016. Patients were randomly assigned into two intervention and control groups based on blocking sampling method. In each group the routine daily cares to prevent bed sores were performed by nurses. In the intervention group in addition to routine nursing care to prevent bed sores, twice a day (hours of 9 and 21) pure Aloe Vera gel on the areas of hip, sacrum and heel were rubbed, but in the control group placebo (gel of water and starch) were used. Then sacral, hip and heel of both groups on days 3, 7 and 10 for of signs of pressure ulcers was evaluated. Results The mean age of patients in the control group was (42.34 ± 12.19) and in the intervention group Was (41.71 ± 11.50) years, respectively. In the intervention group 1 patient afflicted with sore of hip and two people with sacral pressure ulcer. In the control group 3 patients affiliated with sore of hip, 8 people with sacral pressure ulcer, and 1 person had pressure sore of heel. Analysis of the data showed that both groups had statistically significant differences in the incidence of pressure ulcers (P = 0.047). This means that Aloe Vera gel could prevent the occurrence of pressure ulcers in the intervention group. Conclusion Due to the effect of Aloe Vera gel to prevent a rise in temperature, non-blanchable redness, swelling and pain of the skin of regions under study in hospitalized patients in the orthopedic ward, applying of it toward the prevention of pressure ulcers in patients at risk of pressure ulcer development is recommended. Objective: The protective effect of aloe vera is due to its anti-inflammatory properties. This experimental study focused on revealing the effects of aloe vera on antioxidant systems and determining the number of degenerated neurons after cerebral ischemia and reperfusion. Methods: Twenty-one rats were divided equally into three groups: group I rats were used as controls; group II rats were fed with standard diet, and group III rats were fed with standard diet plus aloe vera for 30 days. For groups II and III, the right middle cerebral arteries were occluded for 45 min and then reperfused for 24 h. After these procedures, rats in all groups were sacrificed. The levels of malondialdehyde and nuclear respiratory factor-1 and superoxide dismutase activity in the right cerebral cortex were measured. The present study was undertaken to evaluate the cardioprotection potential and underlying molecular mechanism afforded by a selenium (Se) polysaccharide (Se-AVP) from Aloe vera in the ischemia-reperfusion (I/R) model of rats in vivo. Myocardial I/R injury was induced by occluding the left anterior descending coronary artery (LAD) for 30 min followed by 2-h continuous reperfusion. Pretreatment with Se-AVP (100, 200, and 400 mg/kg) attenuated myocardial damage, as evidenced by reduction of the infarct sizes, increase in serum and myocardial endogenous antioxidants (superoxide dismutase (SOD), glutathione peroxidase (GSH), and catalase (CAT)), and decrease in the malondialdehyde (MDA) level in the rats suffering I/R injury. This cardioprotective activity afforded by Se-AVP is further supported by the decreased levels of cardiac marker enzymes creatine kinase (CK) and lactate dehydrogenase (LDH), as well as the rise of myocardial Na+-K+-ATPase and Ca2+-Mg2+-ATPase activities in I/R rats. Additionally, cardiomyocytic apoptosis was measured by terminal-deoxynucleotidyl transferase-mediated nick end labeling (TUNEL) staining and the result showed that the percent of TUNEL-positive cells in myocardium of Se-AVP-treated groups was lower than I/R rats. In conclusion, we clearly demonstrated that Se-AVP had a protective effect against myocardial I/R injury in rats by augmenting endogenous antioxidants and protecting rat hearts from oxidative stress-induced myocardial apoptosis.

Statement of the Problem: Herpes simplex virus (HSV) infection is one of the most common and debilitating oral diseases; yet, there is no standard topical treatment to control it. The extract of Aloe vera leaves has been previously reported to have anti-inflammatory, antibacterial, and also antiviral effects. There is no data on anti-Herpes simplex virus type 1 (HSV-1) activity of Aloe vera gel.Purpose: This study aimed to evaluate the anti-HSV-1 activity of Aloe vera gel in Vero cell line.Materials and Method: In this study, gel extraction and cytotoxicity of various increasing concentrations of Aloe vera gel (0.2, 0.5, 1, 2, and 5%) was evaluated in Dulbecco’s Modified Eagle Medium (DMEM) containing 2% fetal bovine serum (FBS). Having been washed with phosphate buffered saline, 50 plaque-forming units (PFU) of HSV-1 was added to each well. After 1 hour of incubation at 37°C, cell monolayers in 24 well plates were exposed to different increasing concentrations of Aloe vera gel. The anti-HSV-1 activity of Aloe vera gel in different concentrations was assessed by plaque reduction assays. Data were analyzed by using One-way ANOVA.Results: The cytotoxicity assay showed that Aloe vera in prearranged concentrations was cell-compatible. The inhibitory effect of various concentrations of Aloe vera was observed one hour after the Vero cell was infected with HSV-1. However, there was no significant difference between two serial concentrations (p> 0.05). One-way ANOVA also revealed no significant difference between the groups. The findings indicated a dose-dependent antiviral effect of Aloe vera.Conclusion: The findings showed significant inhibitory effect of 0.2-5% Aloe vera gel on HSV-1 growth in Vero cell line. Therefore, this gel could be a useful topical treatment for oral HSV-1 infections without any significant toxicity.Key Words: Aloe vera; Herpes Simplex Virus-1; Herbal Medicine; Antiviral; Cell Culture

Clinical efficacy of an Aloe Vera gel in preventing traumatic ulcers in patients with fixed orthodontic appliances
Traumatic oral ulceration (TOU) is one of the most common side effects of orthodontic treatments. The objective of this trial is to compare the clinical efficacy of an 80% Aloe Vera gel, prepared using a master formula, versus a commercial 0.12% Chlorhexidine (CHX) gel for TOU prevention in participants wearing fixed orthodontic appliances. This report represents a single-centre, university-based, double-blinded, randomized controlled trial with 2 parallel arms. Patients aged 12 years or older, in the permanent dentition, and about to start fixed orthodontic treatment in this university setting were randomly allocated to use either Aloe Vera or CHX gel, following the cementation procedure. Pre-treatment and 1 month after the cementation clinical assessment and digital photographic images were taken of the teeth and assessed by 2 clinical assessors for the presence or absence of TOUs. A total of 140 were randomized and completed the trial. The overall prevalence of TOUs was 43.6%. Background: Intensive care units (ICUs) admissions due to sepsis represent 2 to 11%. Although there are well stablished therapeutic available, it still is the major cause of death in most ICUs and the leading cause of acute kidney injury. The objective of this work was to evaluate the role of Aloe vera extract on survival rates and kidney histopathology of mice submitted to sepsis induced by cecal ligation and puncture. Methods and Findings: Twenty six mice were distributed into four groups: Sham - surgery simulation (N=5); Control - Cecal ligation and puncture was performed (N=7); Aloe - Surgery simulation and treatment with Aloe vera extract (N=7); and Sepsis + Aloe - Cecal ligation and puncture was performed followed by treatment with Aloe vera extract (N=7). The animals were observed until death. Kidneys were analysed by histopathology. Kruskall-Wallis test was used to compare the histopathological results. Survival analysis was assessed by Kaplan-Meier statistics and compared using log rank test. A significance level of 5% was adopted. The survival time ranged from 19 hours for the Control group up to 24 hours for the animals of Sepsis + Aloe group, corresponding to a survival time of 1.26 times greater (p0.05). Conclusions: Aloe vera extracts delays mortality but does not attenuate kidney injury in sepsis induced by cecal ligation and puncture in mice.

AIM: Long-term dietary intake of non-caloric fiber, such as Aloe vera gel influences the structural and activity of micro-organisms in the human gut. The present investigation was executed with an innovative concept: symbiotic effect of probiotics that are used commercially as lactic acid bacteria i.e. Lactobacillus fermentum and Aloe vera juice as prebiotics for a prospective prophylaxis. MATERIALS AND METHODS: Fermentation of L. fermentum with Aloe vera juice certified by International Aloe Science Council was carried out and the quantification of short chain fatty acids (SCFAs) from the fermentation broth in large scale was determined by gas-chromatography-mass-spectrometry selective detection in the selective ion monitoring mode. RESULTS: The growth of L. fermentum and L. helveticus with Aloe vera juice (AVJ) individually on MRS broth was continued to keep at pH 3.5 and 3.6, while pH of the negative controls changed to 4.3 and 4.0, respectively, during 24 hr incubation. The growth rate and the viability of L. helveticus incubated with different concentrations (5-25%) of Aloe vera juice were strongly reduced. However, the growth rate of L. fermentum was enhanced in a concentration dependent manner with emphasis on the use of 15% AVJ that resulted in two times more growth than that of the negative control. Continuation of L. fermentum growth at pH 3.6 in combination with AVJ during incubation for 24 hr suggests the durability of prebiotic potential by AVJ in in vitro fermentation. Acetic, propionic and lactic acid as SCFAs in the ether extract were identified from the fermentation culture medium. CONCLUSION: The prebiotic activity of AVJ may be assessed by the participation of SCFAs during 24 hrs-incubation with L. fermentum. An innovative concept of symbiotics: a combination of AVJ and L. fermentum, is a perspective prophylaxis on future intestinal health claims. Due to tolerance to acid, L. fermentum may pass through the gastric barrier and colonize the intestine after oral administration.

A plethora of publications have reported several cytotoxic effects associated with chronic consumption of thermoxidized palm oil. This research aims to investigate the effects of garlic and Aloe vera on liver function and hepatic cytoarchitecture in rats fed thermoxidized palm oil diet.,Thirty-five male albino Wistar rats weighing 150-180 g were used for this study. They were randomly assigned into five groups (n = 7): control, thermoxidized palm oil diet fed (TPO), TPO plus garlic juice (TPO + G), TPO plus Aloe gel (TPO + A) and TPO plus garlic/Aloe gel (TPO + G + A). The TPO diet was prepared by mixing 15 g of cooled thermoxidized palm oil with 85 g of rat feed. The juice and gel were orally administered at doses of 2.00 ml/kg and 19.12 ml/kg, respectively. After 3 months of feeding and administration, the animals were sacrificed using standard methods and blood collected via cardiac puncture for analysis.,Serum alanine aminotransferase (ALT), aspartate aminotransferase (AST) and alkaline phosphatase were significantly increased (p < 0.001) in the TPO group compared to the control. This increase was reduced (p < 0.001) in all the treated groups compared to TPO, with the greatest decrease (p < 0.001) seen in TPO + A. Total bilirubin was increased (p < 0.001) in the TPO group compared to the control, whereas there was a significant decrease (p < 0.001) in all the treated groups. Serum proteins and plasma fibrinogen were lowered (p < 0.001) in the TPO group compared to the control but increased progressively in all the treated groups. TPO induced prominent histopathological derangements of the liver tissues. However, there were marked improvements following treatment with garlic and Aloe vera.,The results obtained in this study have revealed that chronic consumption of thermoxidized palm oil is hazardous to health by inducing hepatotoxicity, as seen in increased ALT, AST and total and unconjugated bilirubin and decreased total protein. However, these debilitating effects were seen to be greatly ameliorated following garlic juice and Aloe vera gel administration. If these results are to be extrapolated to humans, then the chronic consumption of thermoxidized palm oil diet should be seriously discouraged.
