Effect of Hyaluronic AcidTrehalose in Two Different Formulations on Signs and Symptoms in Patients with Moderate to Severe Dry Eye Disease
Purpose. This randomized, observer-masked, crossover study investigated the effect of two hyaluronic acid/trehalose-based containing formulations, with different physical properties, on the signs and symptoms in patients with moderate to severe dry eye disease (DED). Methods. In one group, patients received a mixture of sodium hyaluronate and trehalose (HT, Thealoz Duo®) for use during the day. In the other group, patients received a more viscous formulation consisting of hyaluronic acid, trehalose, and carbomer (HTC-gel, Thealoz Duo Gel) to use pro re nata. Both groups used HTC-gel before going to bed. Clinical standard tests for DED were performed at the beginning and end of each one-week period. Further, patient satisfaction including quality of sleep was assessed using a visual analogue scale. Results. Corneal fluorescein and conjunctival lissamine green staining scores decreased, and tear breakup time (BUT) increased for both groups ( each). Mean instillation frequency was 3.1 ± 2.6 drops/day when using HT and 1.9 ± 2.2 drops/day when using HTC-gel (). A significant improvement in the quality of sleep was observed with both treatments (). Conclusions. Our results show improvement in signs and symptoms of DED in both groups. While instillation of HTC-gel resulted in a lower instillation frequency, both formulations of trehalose showed good clinical efficacy. This trial is registered with NCT02980913.

Nutrition has gained tremendous respect thanks to the ability of these key nutrients to support long-term eye health. Interestingly, over time, our food system has become flooded with processed foods devoid of nutrients, and our produce has become less nutrient dense thanks to commercial farming practices. While food was once enough for healthy eyes and vision, it is now more difficult to achieve eye health while aging without attention to these key nutrients. Conditions like gluten sensitivity and celiac that can cause malabsorption even further exacerbate the need for attention to nutrition and supplementation in order to support long-term eye health and vision. One study enrolled 3,640 patients with AMD and declining visual acuity to administer high-dose vitamins C and E, beta carotene, and zinc supplements. Comparison with the placebo group demonstrated a statistically significant odds reduction for the development of advanced AMD by supplementing with the antioxidants plus zinc. The study followed these patients for up to nine years. Another study evaluated the effect of multivitamin supplements and vitamin E on patients at risk of nuclear opacification. In regular users of multivitamin supplements, the risk of nuclear opacification was reduced by one third. In regular users of vitamin E supplements and persons with higher plasma levels of vitamin E, the risk was reduced by approximately half. Another large, randomized study found that women taking B vitamins (including folate and vitamin B12) reduced their risk of early macular degeneration by more than a third after seven years. Background Transforming growth factor (TGF) family members play important roles in the regulation of corneal integrity, and the pathogenesis of corneal fibrosis. Currently, there are no effective agents targeting TGF-β signaling to diminish corneal fibrosis. Glucosamine (GlcN), which is widely used in the treatment of osteoarthritis, abrogates the morphologic effects of TGF-β2 on retinal pigmented epithelial cells in a mouse disease model. Here, we sought to determine whether GlcN would exert beneficial effects against TGF-β1-induced corneal fibrosis. Methods In human corneal fibroblasts (HCFs) treated with GlcN, the expression of Krüppel-like factor 4 (KLF4) and its downstream signaling effects were determined in the presence and absence of TGF-β1 using immunoblot analysis. We further explored GlcN inhibition of fibroblast-to-myofibroblast differentiation via KLF4 siRNA. The effect of cycloheximide on KLF4 protein levels with or without GlcN administration was assessed to determine whether GlcN affects the stability of the KLF4 protein. Results In HCFs, GlcN induced the expression of KLF4, which regulated the maturation and maintenance of the ocular surface. GlcN partially suppressed the TGF-β1-induced expression of alpha-smooth muscle actin (α-SMA) and reduced the collagen contraction capacity in HCFs, suggesting a decrease in fibroblast-to-myofibroblast differentiation. This effect appeared to be mediated through suppression of Smad2 phosphorylation and ERK-dependent signaling. The levels of KLF4 mRNA were increased by GlcN and decreased by TGF-β1 and the TGF-β1-induced α-SMA mRNA expression was upregulated when the KLF4 gene was silenced. GlcN also appeared to stabilize the KLF4 protein, reducing its turnover in corneal fibroblasts. Conclusion These findings shed light on a novel mechanism by which GlcN suppresses TGF-β1-induced fibroblast-to-myofibroblast differentiation through the upregulation of KLF4 expression. Current strategies for treating corneal fibrosis were not effective. Elevating KLF4 levels through the use of GlcN might provide an effective alternative to alleviate the development and progression of corneal fibrosis.

These include retinopathy, cataract, dry eye, and pseudotumor cerebri: Retinopathy: characterized by yellowish to white lesions studded with tiny holes in the peripheral retina. It is typically caused by a deficiency in vitamin A. The vitamin A deficiency can also lead to night blindness (nyctalopia). Typically, visual function improves after one to four months of vitamin A supplementation. Cataracts: marked by the clouding of the lens of the eye, most cataracts develop slowly over time and cause blurry vision. Cataracts can be caused by a vitamin D deficiency, as a vitamin D deficiency interferes with the absorption of calcium and the resulting hypocalcemia (low blood levels of calcium) contributes to the development of cataracts. In addition, diarrhea common in undiagnosed celiac patients can lead to dehydration. Dehydration can alter the composition of the lens and lead to low levels of calcium in the aqueous humor. This leads to hypocalcemia which can then lead to cataracts. Pseudotumor cerebri: also known as idiopathic intracranial hypertension is characterized by increased intracranial pressure of unknown cause. It is thought to be caused by nutritional, metabolic, endocrinological or hematological disorders. In particular, it is linked with both vitamin A poisoning and vitamin A deficiency, although the mechanisms by which vitamin A deficiency and poisoning results in increased intracranial pressure is still not fully understood. Vitamin A deficiency is a common manifestation of gluten induced intestinal damage. Dry eye: just as it sounds, dry eye results in uncomfortably dry eyes and sometimes small spots present on the whites of the eyes called Bitot’s spots. Vitamin A deficiency is often the root cause behind this condition as well, and research has shown it is more common in celiac disease patients. I started to losing vision in one of my eyes. It was going blurry and I´ve problems before, I´ve cataract surgery and I did two things for my eyes, for that eye. I would spray a diluted solution of magnesium chloride (magnesium oil) with water like 15, 20 parts water per one part magnesium and I took my hydrogen inhaler, which one of the ones I´m using from Vital Reaction (there are a big unit) puts out 4 liters a minute so you can actually feel, you can feel the flow and I held up to my eye and just kind of when you put out a fire and just spray for a few days with the hydrogen. So, I´m feeling better. I went to eye doctor and he said it's going to get better by itself now, and it did. So, he didn't do anything and I can swear and there is no way to say which of the two helped or both together. I would think, both together. I am doing this video to a company and short writings of mine from my Hydrogen Medicine book on the research that is available that finds that - yes, hydrogen gas does have an effect on the retina and have neuroprotective properties by taking down the oxidative stress and inflammation. Hydrogen is unique is this regard in terms of being an antioxidant that gets, because it's so small hydrogen gets everywhere. There is nowhere that we can´t get. So you take something like peripheral like your toes or your fingertips or your retina and your are at the extremity, you are extreme of the ability of hydrogen to get to these locations. So flooding the body with hydrogen or in may case, actually blowing hydrogen gas right on the eye, is good medicine, because we know when you are inhaling hydrogen it will, and that's what the studies are, it will reflect to the extremities, will reach the retina. So, if you are going blind and your doctor needs to prescribe an antioxidant because you are totally, you know oxidative and stress levels through the roof, think about hydrogen and yes, think about getting some magnesium oil and spraying in the eye. It is very soothing, just it stings a little bit and that is why you can control the dilution. You can start out with a higher dilution and then make it more concentrated with the time. Your eyes gets used to it because it´s salt. Magnesium Chloride is a salt, so kind of burns a little bit, but it feels good. Some other people have had success with improving their eyesight, people I know, so, this is a legitimate evidence-based scientifically based application for hydrogen medicine, specifically hydrogen inhalation. In the water of course will reflect more to the intestinal tract.
